  Mail:  The completed form can be mailed to the address provided on the form.
*Note:  The Mail option must be used for Architectural Request forms.

  Fax:  The completed form can be faxed to the fax number provided on the form.

  Electronic:  The completed form can be scanned in Adobe or Word document format and emailed as an attachment to the e-mail address provided on the form.
